Without my Governor Doyle's assistance, I would still be seeking diagnosis after six months of being ignored. I politely asked if we could discover why all those communications from my doctors seem to disappear, and why there was inefficient flow communication. I was told to write a letter. I wrote to the County Executive Scott Walkers office, it  seemed appropriate as he is  in charge of the Milwaukee County. Also he was on  a search for money, by slashing budgets,  how better to find money than  to use an existing budgets, but rather use this money more efficiently!

Since I was given no answers I decided to use my legal right to legally request my own patient information within the gamp bureaucracy, and so I sent them is digitally signed and legal document three times now requesting my patient information. Gamp recognized receipt of this legal document, yet  denies it as any paperwork concerning me, they're patient. I told them what I thought of that analysis, as you can read below.

If I must fight them in the courts to determine why things are so inefficient I will, hoping those that follow me will get the support that GAMP was intended to provide as a core function of the community as Milwaukee County Executive Scott Walkers assistant states.

The 31 emails, starting in the Governor Doyle administration, onto General Assistance Medical Program - Milwaukee (GAMP), and then on to the Milwaukee County Executive Scott Walker administration, and eventually complaints to the federal government. 

 

 It is a joke, except the inefficiency and waste are worth crying for.

I got in touch with Audrey at GAMP, but did not get very far with her.  She told me that Vince has not chosen a provider or a clinic and that is why there is a hold-up, but it seems from his website that he is seeing GAMP-approved doctors in GAMP-approved clinics (such as Dr. Wetzler and Dr. Deckard).  I am most interested in Dr. Deckard at this point because he seems to be the kind of specialist that Vince needs in order to nail down a true diagnosis, and get on the path to effective treatment or surgery.  Am I correct that Dr. Deckard is GAMP-approved?  If so, have you had any updates on the tests that Dr. Deckard recommended?  Have they been approved yet, or is Vince still waiting?

Would it be better for me to contact Vince directly?  If I can get the most recent updates on doctor's visits, tests, etc. from Vince, I will then try to contact someone (other than Audrey) at GAMP and DHFS to follow up.

I was assigned to Audrey two years ago - and never found any cooperation from her.  When I went to St Mary's Emergency room, they called her on my behalf, and worried they may have caused me harm by pushing her to anger. I am not sure if her attitude would be improved with a well deserved vacation, or a job requiring yearly reviews; but her boss is totally cool.

The last time Audrey stalled out, I found a friend who knew her boss, Kathy I believe.  Kathy found my approval sitting on a pile to be faxed, just sitting there for nearly a week, and Kathy personally took over Audrey's task so I could make the appointment with Dr Deckard.

I believe that Dr. Deckard normally does not accept GAMP, but did so for me as a professional favor to Dr. Wooten.  I know he was concerned that GAMP usually will not approve the expensive tests he wants and needs.  I am still awaiting any answer, yes or no.  The web site is up to date.

The no clinic non-sense is either a butt saving maneuver or Audrey is ignorant.  Dr Wetzler has no clinic, yet this is well known to Audrey's boss, and so likely Audrey too, and Kathy (Audrey's boss) considers him, Dr
Wetzler, one of the best GAMP doctors in their system because he cares and puts up with all the bureaucracy and no money.  There is no 'proper' clinic because I have followed Dr Wetzler through four different clinics, all over town.  Dr Wetzler has called the problem 'political - medical people trying to deal with non-medical bureaucrats'.

Please tell me when or how to communicate directly with you so I can help you help me in any possible way. What else can I do, answer, research, or whatever to help?

As you, and so now my governor too, are likely examining my situation
in some detail, as well as how it relates to GAMP-Milw., for the sake
of others, please explore a tragic inequity systemic within this
system, just bit further, and explain to me the actual law, and rules
because on the bold face of it, it seems to be clearly
unconstitutionally applied.

As an independent contractor, I was dis-qualified for GAMP assistance
last year, on the basis of my Gross Income, and was also forced to
supply another tax return to show cause for further consideration of
eligibility, which was not legally an option to me until end-of-year
taxes were acceptable by the IRS.  I was struggling to eat, with
almost no gross income, and thousands of dollars of phone bills to
NY, as I struggled to salvage the long term contract with the biggest
bank in the world, because my contract with them exploded with the
9-11 WTC building failures; specifically Liberty Plaza, DB, NA.

As a typical employee, I was told, it would have been easy for me.
Supplying the next and or last two pay stubs would have been enough
for reconsideration of eligibility, and net income, not the gross,
would have been used as the basis criteria; net income is money
usable for food and the like. I was hungry.

Self-employment is not a contagious disease worthy of scatter by
society, it must be embraced as our future; self-employed adventures
should not be a labeled as activities from second-class human
involvement:  GAMP assistance is not equitable, it is not uniformly
distributed or employed, can it possibly be legal?  Was Audrey at
GAMP absolutely fair and correct in application of these GAMP laws?

I thank you very much for the introduction you gave me within GAMP.  Bridget Murray invited to call her anytime, and she is kind, helpful, and concerned, yet there is a communication problem in the UR department.

The Utilization Review department has been contacted several times by Dr Deckard's office on my behalf to request the diagnostic MRI tests Dr. Deckard's needs to make a sound assessment.

After my office visit on the 16th of June, the doctors office faxed a request on the 18th, called Lori on the 25th to follow up, and again faxed a request that same day;  then again left a message and faxed a request on July 1, and again faxed a request this morning July 2.

This seems like a lot of wasted time and energy for the specialist doctor, who is doing us (me) a professional favor.  I have left a message with the UR department requesting a return call to explain the status and all specifics this communication, and their phone number is 414.257-6994. (Bridget said to speak to anyone who answers the phone - no one did)

I also obtained blank forms to authorize release of patient information which I will happily fill out and send you if that may help dig into the specific details.  Otherwise please feel free to do anything that come to mind if you suspect it may help, with my blessings and lively gratitude.

Thank you again for you assistance, you helped me, I am being treated respectfully and promptly.

The GAMP UR department did call back, and told me according to the computer, today they first received a request for my needed MRI scans, and so they needed 72 hours to review the situation.  I asked if we could please consider the 72 hour period beginning as from the 18th of June, the first fax request, or the 25th, the second fax, or the 1st of July, the third fax request.  I was told nothing came until today.

I requested they please look into the matter with an informal investigation to understand what happened to all the communication between Lori and Dr. Deckard's office so I could avoid such delays in the future.  I was told to write a letter to request any investigation.  Then I suggested that common courtesy required that I keep the Governors office informed about this situation as you are involved, and expecting word from me.

Within less than one minute, they found it in the computer, the request 'had been approved', and they promised to immediately fax it to Dr Deckard so I could begin to schedule these tests today.

Using your office as a bully pulpit was amazingly effective, but what I was told does not logically ring true.  Should I formally write a letter to request  that this matter be investigated further, because this is what happened last time too, or might such a letter be pushing too hard for my own good.

I really wish to eliminate such costly delays in the future for myself and everyone else too, including the doctors office, which surely has better things to do.  Please advise me, what do you suggest I should do next.

I was so delighted that DR Deckard agreed to help me as a professional favor/referral, he gave me a huge favor.  I am embarrassed, insulted, and furious with the way GAMP dealt with him.  Why should this specialist doctor help me for no money, piles of paper work, and the run-around; why would GAMP treat him like with complete disrespect.  I feel betrayed.

9.  07-04-03 GAMP investigation warranted and requested

My sister asked for a favor, and found an ear at the Governors office. Erin, who thankfully came to the rescue with care, professionalism, and effectiveness, found my GAMP specialist who blamed the delays on me. Erin researched at the web site, and saw past this ridiculous statement, found her boss, the head of GAMP specialists, who was great, and her people told us, that my doctor made no requests of them, which was proven to be non-sense. This was the second of three un-truths, intended to evade responsibility.

Next someone in the Utilization department dropped the ball again, lied to me, refused to give her name, but suggested I write if I have a problem, so I decided to oblige her. I don't care if she likes me, perhaps a taste of fear - my fear - will help.

My friends ask me not to make waves - but I don't see how it matters much, this should not be an issue of favors, but rights. GAMP is ignoring me as nothingness, if I seriously annoy them, can they harm me any better, so 'Dam the torpedoes, full speed ahead.'. Some inept bureaucrat in the GAMP Utilization Review Department cares about themselves, but not the sick they are employed to help.

The Utilization Review Department failed to act professionally twice, just with Dr. Deckard.  This department stalled to approve the first doctor consultation, and then abused the system to delay progress to explore my problem, why? The details are below. (see 07-03-03)

There was another personal favor to me involved here too.  I was so delighted that DR Deckard agreed to help me as a professional favor/referral, he gave me a huge favor.  I am embarrassed, insulted, and furious with the way GAMP dealt with him.  Why should this specialist doctor help me for no money, piles of paper work, and the run-around; why would GAMP treat him like this, with complete disrespect.  I feel betrayed.

Power comes from knowledge, timing, and its' assumption, I assume. If I don't fight for myself and my doctors, why should anyone else care if I live or die.

The letter I wrote, as requested by the lady in the GAMP Utilization Department, who would not identify herself, said in part:

OK, ... I decide to I demand any and all legal rights due me to INVESTIGATE this situation, to my complete, detailed understanding.  If it is within my legal right to act as my own medical authority, I write now to all of you to demand an audit of every snippet of information concerning me, ...  at GAMP-Milw..

Dear Darlene:

Thank you sincerely, for introducing yourself to the situation which
I find so unacceptable.  I apologize if my two requests were too
vague for you to understand, so as you asked below, I will try to
clarify some.  There are two primary issues which I hope you will
find worthy of investigations, also please give me the benefit of the
doubt, I am trying to help, not cause problems.

I request confirmation that the normally-employed vs. self-employed
differences were applied as the law is written, and if so, can this
law or rule, whichever it is, be legal since it discriminates against
self-employed individuals, and in my case has caused me great
problems. Please direct me to the specific law or rule directing
these restrictions for self-employed people.  The self-employed are
at a huge disadvantage with a net vs. gross income, and with respect
to timeliness of care eligibility. Since you tell me that it is as it
has always been, I am somewhat frustrated since such a response does
not address the question.

The second issue you seem to have ignored completely - perhaps you
did not understand that I was asking you to read parts of the web
site I pointed you to.  I request an investigation of GAMP procedures
at the suggestion of GAMP UR Department. Please consider this my
official request to investigate what happened with my account at
GAMP, especially during the period of June 26 through July 3.

For six months I was eligible for GAMP, and during this period my
doctors repeatedly made requests, and these requests were almost
always ignored.  As you will note form my Medical Status Journal, I
requested an informal investigation to determine why so many urgent
requests seem to be lost or just ignored. I was told to write a
letter to make such a request to investigate, This is it.  If  I am
addressing the wrong office as you have no authority over this county
department; please advise and re-direct me as appropriate.

If these problems of efficient information flow and productivity are
honest mistakes, I then must suspect that there are inadequate audit
abilities inherent within the system, and so such problems are likely
common to all patients and GAMP employees alike, systemic through out
the system.

If there is a simple, effective audit trail from the inbound fax
requests, and a description of the efforts, the time, and date of
each person who worked on these faxed requests, surely inefficiency
would improve, and long term costs would be reduced.  Accountability
for accuracy, professional judgment, and productivity must be
considered if such a system does not now exist, especially in light
of the potential for employees to ignore requests so that their take
home pay increases.

Since these inherent conflicts of interest exist, audits of internal
productivity is highly prudent within this governmental organization.
Such audit reviews are commonplace in the private sector to increase
efficiency, and a governmental department needs it too for efficient
operations.  All authority must have commensurate responsibility, and
an auditable production history log will pressure employees to
produce or perform their duties more efficiency.

As you have been in touch with Bridget Murray, Operation Manager for
GAMP, you will likely agree with me that she is kind, effective, and
apparently sincere in her wish to understand 'who dropped the ball'
as she stated it.  I understand that the UR Department is a different
department, not her authority or responsibility, though I could be
incorrect in this assumption.  From what I have seen, I can only
assume that the Utilization Review Department, not Operations, is the
cause of the poor and inefficient information flow.

Please see the following two links to my medical status journal for
more specifics surrounding my request for an investigation. Please
note, It was only due to attention from the Governors office that I
was allowed help, the specialized help I needed. Without this helpful
intervention from the Governors office , nothing salient, directly
addressing my life threatening medical situation, would have been
accomplished within the first six months.  Also, please note that I
politely asked for an informal review of what happened to the lost
requests from Dr. Deckard's office, just so I could improve the
communication flow, and minimize the waste of time, and too, the
damage to my health. I was told to write and formally request an
audit investigation.

2. Is GAMP policy governed by law or policy - please send documentation.

I am confused by your bosses, Scott Walkers, lack of cooperation and
apparent lack of interest enough to address my questions directly,
especially as my county supervisor is running around the county
asking for ideas. I wish only to make each health care dollar produce
more.


Please tell me the pending schedule of these town meetings which
County Executive Scott Walker is hosting, so I can go on crutches, if
I must, read my emails, and ask why I am unable to get answers. I
realize he is very busy and perhaps no one in his office has looked
into the problem as much as is required to understand what I am
concerned about; I have heard nothing to make me believe so.


Milwaukee Counties' medical costs are great and rising fast, even a
small percent increase in efficiency can be significant. I believed
the county supervisor would be eager to look at the problem, as I am,
and intend to find a way to help with or without your cooperation


I suppose I did send the last medical opinion for two reasons, 1) it
was symbolic that it took two years to get back to Dr. Dicus, which
hopefully shows vividly a problem of inefficiency, and 2) to gently
assure you that I will not be dissuaded from contacting you and the
county supervisor with my two simple questions, in order to further
my intention to improve efficiency of the Milwaukee County health
budget and therefore lessen the health care crisis.


FYI: I ask two simple questions and get no direct response, note to I
am offering to help investigate the inefficiency which I believe will
help our county survive these trying times.
   1. Is there an audit trail in place now to track each medical
request and follow it through the processing to decision, and
   2. Is the differing treatment of self-employed vs. normally
employed governed by a law or a rule.

These should be simple to answer, and I am now curious why no one
responded to my last (third) request. If you wish to discuss it more
privately, I will call you whenever you ask. Thank you for asking
though, I need to know you are getting my requests and ignoring me,
rather than wondering if you received them.


Please, I want your support.

Thank you very much for the time required to respond to me directly.  I accept and respect your time is much more limited than the requests upon your time... but I believe I am on your side.

I can appreciate preciousness of time, as I can do so little in a day recently, because I have such a hard time walking, standing or even sitting; yet I must find the attention of my county supervisor, and I am now seriously disillusioned with the lack of direct answers, which I believe were easy to understand, and should have been easy to deal with electronically.

I am also very discouraged that you are making me drag my disabled body into a public forum, when the current day technology should and could have been efficient.  What puzzles me most is that I truly expected we would be on the same political side of the budget efficiency issue.

Please inform my supervisor that I am the skinny, gimpy, red head who is coming to ask questions, and read my email correspondence directed to him, without adequate response: I will ask: what percent of the budget is health related, how fast is it growing, what is the actual dollars spent last year, and why he ignores my inquires for greater efficiency.

Regarding the GAMP program - this is a program for the indigent that do not
qualify for Medicare or Medicaid and have no other health insurance option.
Milwaukee County is not mandated to have the GAMP program, but it was put
into place after Doyne Hospital was closed to serve the indigent
population.  Milwaukee County sees it as a service to its residents that
need health care assistance.  Again this is not a mandated or required
program for Milwaukee County, however, it is a program that is necessary
and speaks to the core functions of county government.

Regarding your question on an audit trail - I would not have this
information.  I am also e-mailing Bridget Murray and asking if she can
address this question for you.

Also, I guess I am confused on your website it states that you are in
excellent health.  If that is not the case, you may wish to change your
website.

Your response was too late for me to attend the last meeting due to pre-scheduled medical appointments; this might have been an oversight on your part, as you say, however if you honestly are requesting answers from GAMP I will accept that as progress, albeit begrudgingly on your part.  By now you and Scott Walker must be able to conclude that I will not stop insisting upon answers because as you say 'it is a program that is necessary and speaks to the core functions of county government'.  I add it must be efficiently administered or the public will not accept your leadership, and it must equitably administer, too, or it is not legal under the American system of law.

In the meantime, as we both wait for honest, forthright, answers from GAMP, I must protest the demeaning and insulting tone of your last email.  I also wish to point out that it is riddled with illogic or untruths, neither of which should be acceptable to an administration that came to power through a re-call election.  I am also inclined to surmise that your lack of answers shows true apathy, not a good sign from Scott Walker and/or his choice of staff.

Regarding your question on an audit trail - I would not have this information.  I am also e-mailing Bridget Murray and asking if she can address this question for you.  You should know by now, and how can it be a confusing answer, unless the answer is no?  It is your duty to discover answers to this and my other questions.

Also, I guess I am confused on your website it states that you are in excellent health.  If that is not the case, you may wish to change your website.  It would take to much time to update my resume since 1990, it is not relevant, and way outside my prioritized list of things to do.  I am however building a web site dedicated exclusively to this issue, it is meant to help you find the pertinent data more easily.

Since you are the first and only who told me to send my 35$ administration fee to continue my GAMP eligibility, you know perfectly well I am not in excellent health. If you read what I sent you, and copy below, you know I am not in excellent health.

I have nearly 160 million bytes of information on the two web sites - 140 Mb on my extremely poor health, all of which you ignored.  Instead of trying to serve the electorate, you searched enough to find an old resume summary which was last updated in 1990 I believe, and chastised me for my errors, explaining you believed my health still to be excellent.  I do not believe you are so stupid or busy, so I believe you are not serious or not sincere. If this is not dishonesty on your part, it surely is incompetence and inefficiency, which is why I came to Scott Walkers office in the first place.  We desperately need increased efficiency of information flow.

I hope this information is of assistance to you.

As  a patient, I have been patiently trying to understand the reasons for the apparent inefficiency of the inner workings of Gamp, General assistance medical program -- Milwaukee.  Until yesterday I had no informative response regarding my request from anyone in authority.  I am truly delighted that you were willing to acknowledge the receipt of my legal, electronically signed, request for the release of my patient information to me, as law insist that you must oblige.
 
As I mentioned to you several months ago when I first started asking you for help, I am an information specialist with considerable experience analyzing information flow.  There is absolutely no way that I can conceive of, which would allow GAMP operations to function as you say.  The General assistance medical program - Milwaukee must have something written down; bureaucrats love paper, they do not destroy it, it is their life-blood.
 
The General assistance medical program -- Milwaukee, is an organization, which behaves essentially as an HMO.  Because, the General assistance medical program -- Milwaukee is my pseudo insurance Co., you must certainly have information, regarding me with my name and my patient ID number on file. This includes both computer records, paper records, telephone logs or records, and the archived faxes.
 
My doctors and I have send you countless messages in the form of faxes (your official and requested method of communication), e-mail, and also several electronically signed e-mails.  It would astound me very much, if you did not have an accurate and comprehensive record of me, Vincent J. Cataldi.
 
How could it be logically possible that your organization is behaving as an insurance Co., and your employees within the gamp utilization department do not have the previous history for each patient in order to determine what is sensible, required or ridiculous?  How could you possibly work within a budget, understand your budget, or request next year's budget, unless you saved this information: how could you possibly hope to be efficient unless you did so!
 
Also, it is not conceivable to me, that you do not have a budgetary analysis per patient at your fingertips, including what was Spent, when, and why; including who made each of the decisions to allow or deny medical service per patient.
 
I ask you please to use a broader syntactical perspective, and reconsider my legal request to release to me my patient records.  If you continue to insists that you have no information concerning me, your patient, you only reconfirm my worst fears regarding your inefficiency, most likely an inefficiency, and too. a systemic inefficiency, through out the General assistance medical program --
Please let us work together, the efficiency of every dollar spent is more critical today than ever.

Also, it is not conceivable to me, that you do not have a budgetary analysis per patient at your fingertips, including what was Spent, when, and why; including who made each of the decisions to allow or deny medical service per patient.
 
I ask you please to use a broader syntactical perspective, and reconsider my legal request to release to me my patient records.  If you continue to insists that you have no information concerning me, your patient, you only reconfirm my worst fears regarding your inefficiency, most likely an inefficiency, and too. a systemic inefficiency, through out the General assistance medical program --
Please let us work together, the efficiency of every dollar spent is more critical today than ever.


 
I am still waiting for answers to simple questions, are the answers a secret?
 
In the meantime, please accept my request herein to send me all my medical records at GAMP, and send these records to me by eFax, eMail or snail mail.  Also, please email me or call me to acknowledge that your are cooperating.  I am now receiving three bills rejected by GAMP - two from St. Mary's emergency room, and one from my GAMP doctor, and all of these are bills incurred during my GAMP eligible period: 1-29-03 to 7-29-03.  This may be nothing more than another example of inefficiency at GAMP Milwaukee, or worse, it could be vindictive retaliation because I am insisting on answers to question which I mentioned above.

As I interpret the law, you are on the wrong side; please review this third request for my patient information, and comply with the law,  It would rub me the wrong way if legal fees were wasted because Milwaukee Executive Scott Walker, and all his subordinates refuse to answer me directly.
